The Home and Community Support Specialist (HCSS) performs supportive tasks in the home of individuals affected by traumatic brain injury (TBI) under the guidelines of the NYS Medicaid TBI Waiver Program.

Minimum Qualifications:

• Must be a high school graduate or equivalent (Diploma or GED).
• Must have completed a Home Health Care Aide (HHA) Certification Course or a Personal Care Aide (PCA) Certification Course.
• Must successfully complete the HCSS Training Program offered through Sibley Nursing.
• Must at all times adhere to all Sibley Nursing Personnel Service, Inc. (SNPS) policies and procedures.
