The Service Coordinator assists the prospective participant to become a waiver participant and coordinates and monitors the provision of all services in the Service Plan. Services may include Medicaid State Plan services, non-Medical federal, state and locally funded services, as well as educational, vocational, social and medical services. The goal is to increase the participant’s independence, productivity and integration into the community while maintaining the health and welfare of the individual.
Minimum Qualifications:
- Requires an individual with a Master of Social Work or Psychology; or a Certified Rehabilitation Counselor, Occupational Therapist, Registered Physical Therapist, Speech-Language Pathologist or Registered Professional Nurse and at least one (1) year of experience providing Service Coordination, information linkages and referrals regarding community-based services for individuals with disabilities and have demonstrated knowledge of community resources.
- Individuals with an equivalent combination of relevant education, training and direct related experience may be considered, as follows:
- Bachelor’s Degree with 2 years experience providing service coordination to individuals with disabilities with referral & linkage to community resources;
- Or a High School Diploma with 3 years experience providing service coordination to individuals with disabilities with referral & linkage to community resources.
- Must have reliable transportation and must also possess knowledge of and accessibility to community resources and knowledge of TBI Waiver Services.
