Sibley Nursing is an approved provider of services under the New York State Department of Health’s Home and Community Based Services (HCBS) Traumatic Brain Injury (TBI) Waiver. Our extensive experience with TBI makes us an ideal choice for better care in this critical area.

Along with providing TBI care, we can assist in other valuable ways, helping to locate appropriate community resources and services, including –

  • Housing
  • Transportation
  • Pre-vocational and educational services
  • Needed medical services
  • Connections and information on key related services

 

Independent Living Skills Trainers (ILST)

Our experienced ILSTs work with individuals who have suffered brain trauma to increase independence at home and in the community, according to each individual’s goals and needs. Areas of assistance can include –

  • Budgeting, organization and money management
  • Transportation and mobility training
  • Community re-integration and socialization

 

Home and Community Support Services (HCSS)

Specially trained HCSS staff assist and oversee personal, home-based and community activities covering –

  • Meal planning, grocery shopping and meal preparation
  • Home organization and maintenance
  • Personal hygiene, health and safety
  • Recreational outings

 

Eligibility Guidelines for TBI Waiver Program

To participate in the Department of Health’s HCBS/TBI waiver program, individuals must be –

  • Medicaid-eligible
  • Between the ages of 18 and 64
  • In possession of a medical diagnosis of Traumatic Brain Injury (TBI)
  • Given a choice to live in the community or a nursing facility
